About the Company

Our US-based healthcare client came to us with a crucial mission – the seamless management of patient, provider, payer, and medical staff information. They seek to enhance communication between patients, their families, and the medical team, fostering superior healthcare delivery.

The client encountered difficulties in locating a dedicated platform for comprehensive patient care coordination and centralized operations. Additionally, they faced challenges in efficiently accessing insurance information and aspired to provide a user-friendly solution for both internal and external users.

Challenges

The healthcare organization faced a range of challenges as they worked to improve patient care and operational efficiency. Managing patient data was a key concern, slowing down responses to patient needs. They addressed this by implementing Salesforce Health Cloud for better patient tracking. Simultaneously, integrating with EPIC (EHR System) proved complex, making it difficult to consolidate clinical patient information in one place.

Enhancing coordination among patients, relatives, and the medical team posed an additional challenge. They also encountered difficulties in tracking insurance details and sought a user-friendly solution for this. Prioritizing automation for patient event reminders and streamlining patient history tracking were key goals. These challenges collectively underscored the organization’s determination to provide enhanced patient care and operational efficiency.

Solutions

In response to the challenge, we introduced Salesforce Health Cloud to our client, a powerful solution that made patient care more straightforward and improved how the organization ran its operations. But we didn’t stop there. We also connected EPIC, an Electronic Health Record system, with Salesforce, giving us access to important patient information. However, that was just the beginning.

We set up a system that made it easy to manage patient and insurance information within Health Cloud. Beyond the technical stuff, we created care teams and plans for patients, complete with a timeline view of their whole medical history that all the care team members could see.

To make things even better for patients, we set up automated processes that kept them informed about upcoming events and their care plan goals, which ultimately improved their health. Finally, we made reports and charts that gave us useful information about patient referrals. This overall approach transformed our healthcare partner’s patient care and how they run things, putting them on the path to excellence.
